To obtain a valid referral, you must have obtained it from your CLSC caseworker. If you don’t have a file open at your CLSC, you can contact the Accueil psychosocial of your local CLSC to make your request.
Referrals from other healthcare professionals affiliated with a CIUSSS or CISSS are also accepted.
No, you don’t need to be referred to access ESSAD’s services. You can contact our team directly to discuss your needs and arrange an initial assessment, or fill out the online form.
To obtain our services, you have two options:
Fill in the online form: fill in the online service request form. This will enable us to gather the information we need to assess your needs and rapidly organize the service.
Call us: You can also contact us directly by phone at 514-353-1479. Our team will be happy to answer your questions, assess your needs and plan the services you require.
To start receiving our services, you will need the following documents:
Health insurance number: Whether you call us or fill out the online form, make sure you have your health insurance number handy.
Tax report: This is optional, but to access available grants, you’ll need to provide your tax report.
Your application will be processed as soon as we receive it. If all the documents are complete and the form correctly filled in, we do our utmost to process it as quickly as possible. However, this may vary depending on the sector concerned.
The attendant does not bring any personal equipment. Please provide the necessary equipment.
Yes, if you opt for a service every 2 weeks, you’ll have the same person. For a service every 4 weeks, this may vary.
