Social geriatric navigator

Full-time, one-year contract with possibility of renewal*.
Répit Ressource provides over 3,500 clients with light and heavy housekeeping services, assistance with meals, shopping, daily living and caregiver respite. Under the supervision of the Assistant General Manager and in collaboration with the Partners Committee, the Social Geriatrics Navigator is responsible for implementing, developing and monitoring the social geriatrics project in the East End of Montreal. Acting as a link between the community, the health and social services network and other players in the area, the navigator helps identify, accompany and support vulnerable seniors in order to improve their care and service trajectory and encourage them to remain in the community.

Identification and coordination of interventions (30%)
– Develop and update tools for identification, intervention and follow-up.
– Collaborate with community and institutional partners to ensure a seamless continuum of services.
– Support the sentinel network and participate in communities of practice.
Support for vulnerable seniors (40%)
– Offer active listening and individualized support.
– Make home visits and refer people to appropriate resources.
Consultation and partnership (25%)
– Work closely with CIUSSS and community partners.
– Lead or participate in neighborhood committees and tables.
Project monitoring, evaluation and development (5%)
– Produce balance sheets and reports.
– Participate in funding applications and continuous project improvement.

Qualifications and requirements
– College diploma in social work, gerontology, health or a related field
– Relevant experience in coordinating or supporting community projects
– Good knowledge of the community environment and the healthcare network
– Valid driver’s license and access to a vehicle
Skills and abilities
– Excellent interpersonal skills and great empathy
– Collaborative leadership, organizational and teamwork skills
– Fluency in French and good knowledge of English
– Rigor, autonomy and adaptability
Working conditions and environment
– Full-time position based in east-end Montreal
– Frequent travel throughout the region
